Citation text line OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to overhead struck-by hazards while working below a load, suspended from a crane, while the Internal Lifting Tool (ILT) components of the crane were not fully engaged: (a) At the Port of Providence: On or about 7/23/203, employees were exposed to overhead hazards while walking and working within a crane hoisting drop zone without the necessary controls to prevent access to this area. (b) At the Port of Providence: On or about 7/23/2023, employees were exposed to overhead hazards during crane hoisting activities where the crane was fitted with an Internal Lifting Tool (ILT) with a malfunctioning proximity sensor that prevented full engagement of the ILT components while lifting a 215-ton load.
