(a) Worksite: On or about 10/23/2019, the employer did not ensure that ground conditions were firm, drained and graded to accommodate the hoisting operation being performed by the Altec Digger Derrick, nor were any outrigger pads used to distribute the load weight over a larger area. (b) Worksite: On or about 10/23/2019, employees were exposed to tip-over type hazards from overloading the Altec Digger Derrick during the hoisting operation of five metal poles ranging from 20 to 30 feet in length. |
