Circumstances where retraining is required include, but are not limited to, situations where: Worksite: Employees were not retrained to recognize hazards in which they were exposed to while working from a scaffold suspended on the underside of the Mount Hope Bridge in Bristol, Rhode Island. Consequently on or about 07/23/2013 an employee fell approximately 75 feet to the ground below and sustained fatal injuries. |
