XXXX XXXX XXXX XXXX, RI XXXX XXXXXXXX XXXX XXXXXX/XX/XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX Consumer Affairs XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX, TX XXXX Dear XXXX XXXX, XXXX XXXX XXXX is a non-profit 501 ( c ) 3 that has been serving the community for 130 years and is currently operating as a combined family service, community action program, and behavioral health organization serving up to XXXX individuals across the lifespan. As of XX/XX/XXXX, XXXX launched as a Certified Community Behavioral Health Center ( CCBHC ), Our mission is to support individuals and families of all cultural backgrounds in their efforts to meet economic, social and emotional challenges and enhance their well-being.
XXXX XXXX ( XXXX XX/XX/XXXX ) is currently served in our XXXX XXXX XXXX program, in which he was enrolled on XX/XX/XXXX. He had a prior enrollment XX/XX/XXXX XX/XX/XXXX. XXXX XXXX is being treated for trauma associated with a history of abuse, including being victimized through XXXX XXXX.
