Jobsite; On or about 6/18/2014, at 1665 Hartford Ave. Johnston, RI 02919, the employer failed to ensure that vehicles were properly jacked when changing tires as evidenced by: 1. The employee did not engage the parking break. 2. The employee failed to ensure that a heavy object was placed behind the back tires, to keep the car from rolling backwards. 3. The employer failed to ensure that the Audi Assist Manual was followed. |
