(a) Worksite: On or about 10/28/24, each employee on a walking/working surface was not protected from tripping in or stepping into holes by covers, where a hole in the concrete floor approximately 6 inches in diameter and 4 inches deep, with a portable fire extinguisher in it, was uncovered. (b) Worksite Basement: On or about 10/28/2024, each employee on a walking/working surface was not protected from tripping in or stepping into holes by covers, where a hole in the concrete floor, approximately 3 feet wide, 8 feet long, and 6 inches deep was uncovered. Omni Property Group, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(4)(ii), which was contained in OSHA inspection number 1668194, citation number 1, item number 1a and was affirmed as a final order on 8/9/23, with respect to the workplace located at 870 Westminster Street. Providence RI 02903. |
