Citation text line 29 CFR 1926.602(c)(1)(viii): A lift truck, equipped with vertical only, or vertical and horizontal controls elevatable with the lifting carriage or forks for lifting personnel, did not have the following precautions to include (A) Use of a safety platform firmly secured to the lifting carriage and/or forks (B) Means whereby personnel on the platform can shut off power to the truck and (C) Protection from falling objects as indicated necessary by the operating conditions: a) Facility at 990 Aquidneck Avenue in Middletown, RI: On or about October 13, 2021, the employer did not ensure that a Doosan 25 forklift used for lifting personnel was equipped with vertical controls elevatable with the forks, had a safety platform firmly secured to the forks and a means whereby personnel on the platform can shut off power to the truck.
