Citation text line OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to a caught-between hazard from working near a vehicle that was performing a backing maneuver into an opening with limited extra space available, without adequate spotting protocols and training in place: (a) At the side gate entrance to 131 Narragansett Ave: On or about 7/25/2019, the employer did not ensure that employees followed safe procedures while using a spotter during a truck backing task with limited clearance in that the truck driver and the spotter did not have visual contact with each other during the operation.
