(a) Warehouse at 617 George Washington Highway, Lincoln, RI: On or about 07-15-2024, employees were exposed to crushing and struck-by hazards while operating the Nissan 40 and Hyster 40 powered industrial trucks without wearing a seatbelt. The employer did not enforce employees to wear a seat belt when operating a powered industrial truck. |
