29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Davenport Department: On or about 10/20/23, the employer did not ensure that employees were protected by adequate machine guarding methods in that Machine D11 was operating with the two door guards in the opened position and the rotating pusher tubes were unguarded. b) Davenport Department: On or about 10/20/23, the employer did not ensure that employees were protected by adequate machine guarding methods in that Machine D13 was operating with one of the door guards in the opened position and the rotating pusher tubes were unguarded. c) Davenport Department: On or about 10/20/23, the employer did not ensure that employees were protected by adequate machine guarding methods in that the Davenport Model B, Machine D16, was operating with the door guard in the opened position. d) Multi-Spindle Department: On or about 10/20/23, the employer did not ensure that employees were protected by adequate machine guarding methods in that Machine M4 was operating with two door guards in the opened position and the rotating pusher tubes were unguarded. e) Gildemeister Department: On or about 10/20/23, the employer did not ensure that employees were protected by adequate machine guarding methods in that machine GM16 was operating with one door guard open. The machine was cut so that a part chute could be installed, but the addition of the chute prevented the door guard from closing completely. f) Escomatic Department: On or about 10/20/23, the employer did not ensure that employees were protected by adequate machine guarding methods in that the Escomatic E3 had exposed moving parts, such as the spinning wire stringer, rotating shafts, and wire guides, exposing employees performing quality control checks to these various moving parts. g) Escomatic Department: On or about 10/20/23, the employer did not ensure that employees were protected by adequate machine guarding methods in that the Escomatic E8 had exposed moving parts, such as the spinning wire stringer, rotating shafts, and wire guides, exposing employees performing quality control checks to these various moving parts. h) Escomatic Department: On or about 10/20/23, the employer did not ensure that employees were protected by adequate machine guarding methods in that the Escomatic E9 had exposed moving parts, such as the spinning wire stringer, rotating shafts, and wire guides. i) Escomatic Department: On or about 10/20/23, the employer did not ensure that employees were protected by adequate machine guarding methods in that the Escomatic E10 had exposed moving parts, such as the spinning wire stringer, rotating shafts, and wire guides. j) Miyano Department: On or about 10/20/2023, the employer did not ensure that employees were protected by adequate machine guarding methods in that the Walker-Turner drill press was not equipped with a chuck guard and protection from flying chips and sparks. k) Miyano Department: On or about 10/20/2023, the employer did not ensure that employees were protected by adequate machine guarding methods in that the Miyano MTV-C310, #2, was not equipped with door guard interlocks. |
